The NHS Friends and Family Test

Access to your medical records. You can now apply to access your medical records online. To do this you will first need to be registered for Patient Online Services. Apply at reception by completing a request form.

If you are contacting the practice to book an appointment, please complete an online consultation (or eConsult), you can do this by visiting our website

If you do not have access to the internet or are unable to complete the eConsult, please call the Bexley eConsult line to book a call back and a member of the team will complete the eConsult over the telephone with you.  You can reach the Bexley eConsult team on 020 8269 3406.  Lines are open from 8.15am until 5.30pm – Monday to Friday.

  • It is also used to help us research new treatments,
  • Decide where to put GP clinics and plan for the number of doctors and nurses in your local hospital.

Wherever possible we try to use

  • Data that does not identify you, but sometimes it is necessary to use your confidential patient information.

What is confidential patient information?

Confidential patient information identifies you and says something about your health, care or treatment. You would expect this information to be kept private.

Information that only identifies you, like your name and address, is not considered confidential patient information and may still be used:

For example,

  • To contact you if your GP practice is merging with another.

Who can use your confidential patient information for research and planning?

  • It is used by the NHS, local authorities,
  • University and hospital researchers, medical colleges and pharmaceutical companies researching new treatments.

Making your data opt-out choice

You can choose to opt out of sharing your confidential patient information for research and planning. There may still be times when your confidential patient information is used: for example:

During an epidemic where there might be a risk to you or to other people’s health. You can also still consent to take part in a specific research project.

Will choosing this opt-out affect your care and treatment?

No, your confidential patient information will still be used for your individual care.

Choosing to opt out will not affect your care and treatment. You will still be invited for screening services, such as screenings for bowel cancer.

What should you do next?

You do not need to do anything if you are happy about how your confidential patient information is used.

If you do not want your confidential patient information to be used for research and planning, you can choose to opt out securely online or through a telephone service.

They provide free, friendly and effective practical support as well as offering the full range of evidence-based smoking cessation medications. They also advise on vaping and can direct you to local Vape Shops.

Improving access to general practice: evening and weekend appointments now available

All patients registered with a Bexley GP (or resident in Bexley but have yet to register with a Bexley GP Practice) can now book an appointment to see a GP on weekday evenings (between 6.30pm – 8.00pm) or at the weekends (on Saturday and Sunday and Bank Holidays 08.00-20.00).

Appointments will take place at either Erith Hospital, Main Entrance Park Crescent, Erith DA8 3EE or Queen Mary’s Hospital, B Block, Frognal Avenue, Sidcup, Kent DA14 6LT.

Booking is easy - patients should contact their own GP Practice in the usual way during normal operating hours, and ask for a weekday evening or weekend appointment. To book after the GP has closed, call 111.

To Cancel an appointment if your GP Practice is closed - Call:

0208 269 4120 for appointments booked at Queen Mary's Hospital

01322 356150 for appointments booked at Erith Hospital

By March 2019 everyone in England will benefit from access to general practice appointments in the evenings and weekends at a time that is most convenient to them. ou will need to register for this service at reception and obtain a token advising of your first login details. You can also request access to your journal online by application at reception. If you have any questions ar queries regarding your medical journal and entries within it, you should make an appointment to see or speak with a clinician. Please do not ask reception as they are not qualified to answer such questions.

Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.

Access

A ramp and double doors are provided at the entrance to the surgery for ease of access. If you require any assistance please ask at reception. The patient toilet  allows for a wheelchair and has disabled facilities.
